Through comprehensive and continual assessments, the patient is engaged in the treatment process. Services include assessment, treatment plan and individual and we will be providing group counseling. We provide services for patients who want to learn how to recover from alcohol or other drug use disorders. We will assess and refer to an appropriate level of care from detoxification to outpatient counseling or residential treatment and we provide case management services to help patients learn how to re-engage back into mainstream society. We deliver substance use disorder treatment assessments and counseling that require weekly to bi-weekly attendance at a clinic or facility, allowing the patient to return home or to other living arrangements during non-treatment hours.
Medication Education & Medication Management
Humboldt Park Health Outpatient Behavioral Health Clinic wants to educate for our patients by providing information, education and giving the patients the option to decide about how they want to approach their health and also to be part of the process of getting healthier day by day. Our team will talk with the patients about medicines in a way that is meaningful to them as a person with mental health, we will educate the person about their options about side effects experienced from the medicine and we will ask how they are doing on medication treatment through weekly or bi-weekly counseling. We want to help patients understand the importance of taking the exact dose prescribed to help eliminate hospitalizations. Mental health disorders require a two pronged approach: medication combined with therapy.
