Rehabilitation Services

Humboldt Park Health offers adult inpatient and adult outpatient rehabilitation services. Rehabilitation services is a multi-disciplinary team made up of licensed clinical staff who are experts in optimizing your function to improve your quality of life. Our team provides physical, occupational, and speech therapies.

Physical Therapy

Physical Therapy is an area of non-invasive medical treatment that focuses on helping people maintain or regain the best possible level of mobility and physical function. Patients are often directed to the care of a physical therapist when recovering from an injury, a surgical procedure, coping with a chronic medical condition or pursing pain relief.  The primary objective of physical therapy is to optimize a person’s physical abilities and quality of life.

Occupational Therapy

Occupational Therapy is rehabilitation that focuses on ensuring people with an injury or illness can participate in activities of daily living (ADLs) and instrumental activities of daily living (IADLs) with as much independence as possible. It focuses on restoring function in the upper extremity to promote increased functional use needed for ADLs/IADLs and beyond. They can also address issues related to vision and cognition in regards to daily function.

Speech Therapy

Speech Therapy provides treatment for communication, cognitive, and swallowing disorders. Our therapists can evaluate and develop treatment plans for dysphasia and swallowing issues, stuttering, acquired apraxia, dysarthria, and aphasia to improve your ability to communicate with others.

Our outpatient clinic is located in the Humboldt Park Health Professional Office Building,
Suite 505 at 1044 N Mozart Ave, Chicago, IL 6062.

Clinic Hours: Monday-Friday, 8:00am – 5:30pm.
